Two interesting and totally different cases here. They are so different because S. Meck had the paperwork in hand about the student in question and didn't catch the address/legal guardian error. The Indy kid had no "proper" paperwork at all, which makes address/guardian verification by the AD very difficult, unless he/she plans to visit all addresses on the eligibility list for corroboration. And I'll tell you right now that there is no AD in North Carolina that would, or should have to do that.
