I have now seen this done both ways but I am curious as to which way is correct for NC High School rules. I don't have a rule book handy so I'm going to the next best authority: TBR. The question is: Are only the starters eligible for re-entry? Also, Is it legal for a second pitcher to pitch twice in a game.
    Situation: P1 throws first 2 innings, P2 throws 2 innings, P1 throws 2 innings, Can P2 pitch the 7th?

Both starters and subs can re-enter one time

Is it legal for a second pitcher to pitch twice in a game.

As long as the pitcher does not violate the re-entry rule I believe it is allowed

    Legal...

In this scenario, are you subbing the pitchers for each other (taking one out of the game) or are you just swapping positions (short for pitcher)?

    The way I understand it...

If swapping positions, you can do that as aften as you want.

If subbing, you can re-enter each player one time but once you pull a player for the second time, she cannot go back in. So P2 can pitch the 7th inning but P1 is out of the game for good.

Yes, rule changed a couple of years ago. Subs may re-enter one time after they are taken out, but they must go back in the same spot in the lineup they were in before.

In high school they allow courtesy runners for the pitcher and catcher. This is not a sub situation for them unless the coach wants to actually sub for them.

Anyone who has not yet been in the game offensively or defensively may serve as a courtesy runner. Once the runner has been established it must continue to be the same person. After that person participates in the game in any other capacity she may no longer be a courtesy runner (the pitcher/catcher must then run for themselves).

A player may not be used as a substitute for any other player during the 1/2 inning she ran as a courtesy runner unless its an injury situation and no other substitutes are available.

    Interesting question...

If you put the original courtesy runner in the game (not for the pitcher or catcher), can you have another girl who has not played be the new courtesy runner?
