The second baseman should be one or two steps beyond the first base runner keeping him close and in sight. The pitcher should step off the rubber and walk straight toward the first base runner while keeping an eye on the runner at third. If the runner moves outside his line to 1st or 2nd base, he will be out. Do not make an unneccessary throw. IF the third base runner breaks, gun him at home. If he gets off too far, simply turn and run straight at him; now you have a run down.

I saw a similar play 20 years ago:
Runners on first and second. first base runner purposely took a huge lead to encourage a throw from the catcher and let the 2nd base runner advance to third.
The catcher did not throw to first, but instead ran toward the 1st base runner; freezing him. The catcher kept his eye on the runner at 2nd base. the 2nd base runner kept inching off toward third. The catcher suddenly turns and runs toward the 2nd base runner. The catcher runs perpendicular with the base line straight at the runner, and evenually tags him out- NO THROW.

Worst case, walk the batter, set up the double play. Remember, you have a kid that cannot hit or bunt at the plate. Play the odds. You have a good possiblity of the double play, and the odds are in your favor to get the third out; better than 70 %.
